Opioid Treatment Funds Referral Process

Complete our Martin County Human Services Opioid Treatment Funds Referral Form or contact us to refer yourself or a loved one for treatment. Individuals must complete a substance-use assessment and provide proof of county residency. Must be a Martin County resident for at least 6 months to qualify. Clients will select a treatment facility based on the recommended level of care. Treatment funds are available at the managing entity rate only. While the client is in treatment, the Opioid Case Manager will collaborate with treatment and county resources to develop a case plan.
